We read with interest the article by Maschio et al. [1] on a patient affected by brain tumor-related epilepsy with reversible erectile dysfunction (ED) due to Zonisamide (ZNS). Old liver enzyme-inducing antiepileptic drugs (AEDs), such as carbamazepine, phenytoin, and phenobarbital, are known to commonly provoke sexual dysfunctions by decreasing bioactive testosterone, accelerating sexual hormone metabolism and stimulating binding hormone production. On the contrary, only a few anecdotal reports have described new AEDs-related ED [2,3], although pathogenetic mechanisms remain unclear. We reported on a patient with topira-mate (TPM)-induced ED, in which sexual hormones were within normal range, as well as psychological, neurophysiological, and ultrasound tests [3].
