In the UK, the National Reporting and Learning System (NRLS) is designed to co-ordinate the reporting of patient safety incidents nationally and improve the ability of the health service to learn from the analysis of these events.1 The aim of this study was to examine the reliability of the incident severity ratings used by the NRLS, and explore the likelihood of hospital pharmacists and technicians reporting medication incidents that occur in the hospital setting.
