Improving the safety of epidural analgesia

Epidural analgesia was identified at University Hospital Lewisham as a high-risk procedure for medication errors: preparation, prescribing and administration. Although generally safe when used correctly,1 potential risks are associated in use,Multidisciplinary review led to changes being agreed and implemented in autumn 2001, covering all the processes involved in the procedure. There were five themes: (i) reducing complexity, e.g.. standardising regimes and introducing ready-made bags and syringes; (ii) the use of constraints, e.g. restricting drugs, strengths and volumes; (iii) documentation, including preprinted prescriptions and a standardised pharmacy ordering system; (iv) administration systems; and (v) improved labelling and storage,A reduction in clinical incident reports suggested that the initiatives were successful but the numbers were very small and they are an imprecise outcome measure. This study set up to assess the impact of the changes.Objectives of the study were to obtain the opinions of service users on the safety initiatives introduced, and to identify potential problems in the current provision of epidural analgesia.
