In this issue of the JEVT, the Zurich group1 presents midterm data of a standardized chimney technique performed mostly in patients with juxtarenal aortic aneurysms and some type IV thoracoabdominal aortic aneurysms (TAAA). When analyzing the data, it is important to differentiate between these pathologies. In juxtarenal aneurysms, the chimney technique is more straightforward. There is no need for long chimney grafts, and either balloon-expandable or self-expanding covered stents can be used.2,3 The limiting factor is the length of the chimney grafts currently available. When a length of >5 cm is required, only self-expanding polytetrafluoroethylene parallel grafts can be used.
