To the Editor:HBV infection is a major public health problem. For nearly two decades, vaccines against HBV have been administered routinely and the antibodies they elicit are clearly effective in preventing HBV infections. However, about 5% of healthy vaccine recipients fail to produce protective levels of antibodies (defined as an antibody level <10 IU/l) to hepatitis B surface antigen vaccine after standard immunization. This phenomenon has been observed in all surveys of vaccine effectiveness, irrespective of the HBsAg vaccine employed, and its cause remains largely unknown. Recent evidence showed that CD4~+CD25~+FoxP3~+ regulatory T cells (Tregs) not only could suppress the activation and proliferation of other CD4~+ and CD8~+ T cells, but also could notably suppress B cell Ig production. We tested the hypothesis whether up-regulation of Tregs was involved in nonresponsiveness to HBsAg vaccines.
