Dimensional Thinking and Where to Set the Threshold? Clinical psychiatrists are used to thinking in categories. This is no coincidence: clinicians need to make decisions, the most important of which are binary: "to treat or not to treat?" However, this is no reason for concluding that psychiatric disorders are by necessity binary. Some would argue that many psychiatric disorders are the extremes of a distribution, rather like persistently too high blood pressure or of too high blood sugar levels. How can this be reconciled with what we know about psychiatric conditions?
