Dear Sir, It is time we acknowledge and teach that anti-GQ1b antibodies belong to a spectrum disorder rather than being specific for a specific syndrome. Anti-GQ1b antibodies have been associated with the Miller Fisher syndrome as early as 1950, but the incidence of the Miller Fisher syndrome is less than 1 in a million, while the antibodies are being detected in several other disorders, including the Guillain-Barre syndrome, Bick-erstaffs brainstem encephalitis, pharyngeal-cervical-brachial weakness, acute oph-thalmoplegia without ataxia, oph-thalmoplegia secondary to one extraocular nerve pathology,ophthalmoplegia with bilateral extraocular mononeuropathies, external ophthalmoplegia with areflexia, ophthalmoplegia with normal deep tendon reflexes, or external and internal ophthalmoplegia (pupillary dysfunction).
