Preseptal Cellulitis Secondary to a Dental Infection

A 49-year-old white male presented with preseptal cellulitis of the right eye following nasal endoscopy 24 h earlier. He attributed the cellulitis to nasal endoscopy. A computerized axial tomography (CAT) scan of the head was obtained, which demonstrated a dental abscess extending superiorly into the preseptal space. There was no orbital involvement. PubMed was searched for etiologies of preseptal cellulitis. Preseptal cellulitis, otherwise known as periorbital cellulitis, is defined as an infection of the soft tissue space that is anterior to the orbital septum. Preseptal cellulitis has been attributed to facial trauma, insect bites, local skin infections, and foreign bodies. No cases of preseptal cellulitis caused by nasal endoscopy have been reported; however, a 2-year-old male developed preseptal cellulitis two weeks following oral trauma [1]. Dental infections have been reported to cause orbital cellulitis, but they should also be in the differential for preseptal cellulitis. It is important to perform a thorough dental exam in subjects presenting with preseptal cellulitis.
