Comparative effectiveness research is a central pillar of the unprecedented health care reform that is currently under way in the United States. The priority given to this program recognizes that the nation has by far the most expensive health care system in the world, while achieving a mediocre performance on many key indicators and failing to provide adequate coverage, or any at all, for many millions. The Institute of Medicine's Knowing What Works in Health Care (1) focused on broad potential gains through better alignment of evidence and practice, as did Evidence-Based Maternity Care (2) for care during pregnancy and childbirth. Comparative effectiveness research is viewed as an essential resource for improved decision-making, health care quality, health outcomes, and value for health care investments that are becoming unsustainable.
