A 34-year-old man presented with a 1-week history of headache, nausea and vomiting. He reported untreated hypertension for the last 3 years. On admission, his blood pressure was 280/160 mmHg, and an intense second cardiac sound was observed. Fundus examination revealed papilloedema, retinal haemorrhages and exudates (grade IV retinopathy according to the Keith-Wagener-Barker classification) (Fig. 1b).
