Epilepsy associated with glutamic acid decarboxylase antibody (GADA)

There is increasing evidence from several case series and case reports of the clinical importance of glutamic acid decarboxylase antibody (GADA)-associated epilepsy. It seems that especially temporal lobe epilepsy (TLE) is associated with GADA-associated epilepsy [1,2] as well as with other epilepsies with immunological dysfunction [3,4]. The mechanisms of immune system activation in TLE is partly explained by the particular neurobiolog-ical vulnerability of temporal lobe, abundance of projections of temporal lobe to other brain regions, and the close relationship of mesiotemporal structures and hypophysis-pituitary-adrenal (HPA) axis.This study of Falip and co-workers in this journal introduces the detailed clinical-immunological characteristics of GAD A in patients with TLE [5]. They studied 42 consecutive adult TLE patients: 12% of these patients harbored GADA and 4.8% of the patients had high ti-ters of GADA. In addition, they presented a separate group of seven patients with high titer GADA and TLE. Several autoimmune diseases were found in these GADA positive patients. Previous reports have already suggested the association of autoimmune diseases and other autoantibodies with high titers of GADA.
