Antimicrobial susceptibility and genotypic distribution of Neisseria gonorrhoeae: A 2-year study in Tunisia

Neisseria gonorrhoeae is a major etiological agent of sexually transmitted infections (STI) and may cause severe urogenital complications. Some authors have recently reported a progressive increase in the incidence of gonorrhoeae in many countries. This may indicate a return to high-risk sexual behavior, contributing to the increase of STI, and highlights the need for routine surveillance, prevention, and control measures. Genotyping by pulsed fields gel electrophoresis (PFGE), which is considered as an epidemiological reference marker for several bacterial species, could be used forto N. gonorrhoeae. All the isolates were collected in our laboratory over 2 years (2008-2009), in our region that has a high touristic activity. Antimicrobial susceptibility patterns were determined according to the recommendations of the French society of microbiology. MICs were determined by E-test. The molecular typing was performed by PFGE (CHEF) after macro restriction by Spel. The PFGE patterns were analyzed according to the Tenover criteria. Forty-five isolates were isolated from: vaginal samples (n = 28), urethral samples (n=15), sperm (n=1), and neonatal conjunctival swab (n = 1). Twenty-two isolates were resistant to penicillin, 15 (33.33%) isolates were penicillinase producing N. gonorrhoeae (PPNG), and seven (15.5%) expressed a chromosomalry mediated resistance (CMRNG). Five (11.11%) isolates expressed a plasmid-mediated resistance to tetracycline (TRNG) with a MIC greater than 16 mg/l; two of these carried the combination of both resistance phenotypes (TRNG-PPNG). Fifteen (33.33%) isolates were resistant to ciprofloxacin and four to azithromycin. No antibiotic resistance to spectinomycin and ceftriaxone was observed. Furthermore, three isolates showed reduced susceptibility to ceftriaxone, and only one isolate was resistant to rifampicin. Two out of the four azithromycin resistant isolates were resistant to ciprofloxacin, and the two others were also PPNG.
