摘要： Patient-centered care has over the past decades, been recognized worldwide as an important component of the health system giving a wider dimension to high-quality healthcare and service delivery. The impact on healthcare and services to the patient is the nurses’ ability to create a friendly rapport with the patients. Yet, the majority in the rural Primary Health Care facilities are still facing many challenges in demonstrating patient-centeredness services to the community. Thus, the objective of this study is to explore and describe the factors influencing patient-centered care provision and nurses’ experience in Primary Health Care facilities. An exploratory qualitative approach with purposive sampling was used to gather data. Individual interviews with 35 nurses were conducted using a semi-structure interview guide question in the Primary Health Care settings in Nigeria. Each of the interviews with the nurses lasted for 25 minutes to one hour. All interviews were audio-taped, and transcribed verbatim using Microsoft Word. The transcripts were read and re-read, coded using NVivo version 12 software to organize the relevant information and categorized them into themes and sub-themes. Thematic analysis guided this study. The findings revealed three themes and sub-themes. The impact of environmental factors with two subthemes—suitable working environment and coordinated care;resources—shortage of staff and inadequate resources as sub-themes, and lastly, cultural sensitivity and religious influence—communication. Poor implementation of PCC strategies in most of the PHC facilities could lead to poor patient care and a lack of job satisfaction among nurses. This study identified that nurses have both negative and positive experience in providing patient-centered care health services. Providing patient-centered care in the Primary Health Care setting promotes the goal of achieving universal health coverage in Nigeria if the government would prioritize nurses’ pay, employ more staff, provide a conducive working environment, and opportunities for further training programs for nurses to enable and empower them with the necessary knowledge and skills. This, in turn, will translate into a range of outcomes that are socially valued, such as health responsiveness, health coverage and fairness.

摘要： There is a scarcity of literature discussing nurses’ behaviors toward caring for suspected or confirmed human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) patients. The development of a scale specific to measure nurses’ behaviors will allow health institutions to assess the disposition of their nurses in terms of HIV care. This study aims to present the rigors of developing and validating a reliable instrument to contextualize these nurses’ behaviors. This study utilized a sequential exploratory mixed method design to develop the NB-CSHAP scale. Thematic analysis was done on the qualitative data from the interviews with persons living with HIV/AIDS (PLHA) from which items were selected to be included in the scale. Exploratory factor analysis was utilized to extract the factors and Cronbach’s alpha was used to assess the reliability of the instrument. Four factors were extracted and are categorized as either caring or discriminatory behaviors. These include: (1) service-oriented, (2) openhanded, (3) perceptive, and (4) discriminatory. The scale has an internal consistency of 0.73. The scale shows acceptable psychometric properties, hence can be used to assess the nurses’ behaviors in caring for confirmed or suspected HIV clients. The scale may be used by health institutions to determine the quality of the patient care provided by their nurses to clients with confirmed or suspected HIV .

摘要： BACKGROUND Tube indwelling is a key procedure in modern medicine.Careful tube setup is necessary to prevent unplanned extubation.The training for tube fixation is timeand resource-consuming,and optimal modes of training are currently being sought.Previous studies have compared workshops and flipped classroom models separately using conventional teaching strategies,but no study has examined a combination of both teaching models in nursing training.AIM To compare the effectiveness of workshops vs workshops combined with the flipped classroom model for improving tube fixation training for nursing students.METHODS This was a prospective cohort study.In this study,149 nurses who joined our hospital in 2019 underwent training using workshops combined with the flipped classroom model(experimental group),while 159 nurses who joined the hospital in 2018 received only workshop-based training(control group).The combination of workshops with the flipped classroom training model was divided into two modules:pre-class and in-class training.The participation of nurses in the training activities,onsite assessment of training,nurses’evaluation of their training,and related indicators of tube quality management were evaluated.RESULTS The average age of nurses in the control group was 22.94±0.94 years and that of nurses in the experimental group was 25.42±3.23 years(P<0.01).The qualified rate of after-class assessments for the experimental and control groups was 100.00%(average score:94.01±2.78 points)and 91.82%(average score:84.24±2.94 points),respectively(P<0.01).Most nurses in the experimental group completely agreed that the combined training was helpful to cultivate clinical thinking and independent learning ability and to master knowledge of tube fixation.In addition,the training content within the pre-class teaching video,pre-class tube atlas,pre-class main instructor guidance,in-class demonstration,and in-class practice was very informative.The experimental group had higher evaluation scores than the control group(4.88±0.38 vs 4.67±0.64;P<0.01).Comparison of tube quality management before and after training in 2018 to 2019 revealed that the unplanned ureteral tube removal rate dropped from 0.25‰to 0.06‰,the unplanned chest tube removal rate dropped from 1.07‰to 0.78‰,and the unplanned gastric tube removal rate dropped from 0.36‰to 0.17‰.The incidence rate of pressure ulcers caused by the tube decreased from 0.78‰to 0.45‰.CONCLUSION The combination of workshop and flipped classroom training is effective in improving tube fixation training of nurses,cultivating nurses’active learning abilities and clinical thinking,and improving the safety of the procedure.

摘要： Human caring is regarded as the essence and core of nursing.Based on Dr.Watson′s human caring science theory,nursing staff in Wuhan Union Hospital and other healthcare institutions have delivered warm human caring service in patients′ daily care.During the outbreak of major infectious disease-pandemic of COVID-19,patients are suffering from great psychological distress and pressure.Like nursing professionals around the world,nurses in Wuhan,Hubei,China,along with the colleagues from the National Aid Medical Team,used entire of themselves to conduct diversity human caring measures to patients suffering from the major infectious disease with love and responsibilities,which greatly demonstrated the essence and value of nursing care,and has wide implications for further development of nursing.

摘要： Objective:To explore the effect of the application of health management in nursing on osteoarthropathy.Methods:Fifty patients with osteoarthropathy in our hospital were randomly selected and recruited for the current study from January 2018 to January 2020.They were divided into group A and group B according to the random number table method.group A(25 cases)received routine nursing,while group B(25 cases)received health management in nursing on the basis of the treatment given to group A.Joint function index,quality of life and nursing satisfaction were compared between the two groups.Results:The index of joint function in group B was higher than that in group A,the quality of life in group B was higher than that in group A,and the nursing satisfaction in group B was higher than that in group A(P<0.05).Conclusion:For patients with osteoarthropathy,the application of health management can effectively improve joint function and improve the quality of life.The effect is ideal and the method is worthy of wide application.

摘要： BACKGROUND Nursing officers are an integral component of any medical team.They participate in taking care of basic airway management and assist in advanced airway management,specifically amidst the current coronavirus disease 2019(COVID-19)pandemic.AIM To assess the efficacy of a standardized web-based training module for nurses in preparedness to fight against COVID-19.METHODS The training was held in three sessions of 1 h each,consisting of live audio-visual lectures,case scenarios,and skill demonstrations.The sequence of airway equipment,drug preparation,airway examination,and plans of airway management was demonstrated through mannequin-based video-clips.RESULTS Pre-and post-test scores as well as objective structured clinical examination scores were analyzed using Student’s t-test and the Likert scale was used for feedback assessment.It was found that the mean score out of the total score of 20 was 8.47±4.2 in the pre-test,while in the post-test it was 17.4±1.8(P value<0.001).The participants also felt self-reliant in executing the roles of airway assistant(63.3%)and drug assistant(74.3%).Fear of self-infection with COVID-19 was also high,as 66%of participants feared working with the patient’s airway.CONCLUSION Amidst this COVID-19 emergency,when the health care systems are being persistently challenged,training of nursing staff in the safe conduct of airway management can ensure delivery of life-saving treatment.
