A3-mo-old,12-kg,intact,miniaturepigpresentedwithsevereneurologicsignsonday8afterhematopoieticcelltransplantation.ThispighadreceivedanimmunosuppressiveregimenbeforetransplantationthatincludedanantiCD3immunotoxinforT-celldepletion,100cGyoftotal-bodyirradiation,andcyclosporinefor45d.Thepigbeganexhibitingerythematouslesionsonposttransplantationday7.Healsodemonstratedincreasedconsciousproprioceptivedeficitsandrecumbencybutnormalmentation.Neurologicsignsworsenedoverseveraldays;thepigbecamelethargicbutremainedafebrile.Conjunctivalswellingdevelopedonposttransplantationday9,whichsubsequentlyspreadtotheanimal'shead,earsandhocksbyday10.Analgesicsweregivenforpain,andcyclosporinelevelsweredecreased.Despitethemeasurestaken,neurologicsignsprogressed.Giventheworseningsubcutaneousedemaandneurologicstatus,Escherichiacoliinfectionwassuspected,andtreatmentwithathird-generationcephalosporinwasinstituted.Theclinicalsignsresolvedwithin12hafterthestartofantibiotics.'Shiga-like'toxinfromE.colicancauseperacutetoxemiaandinduceataxia,paralysis,andrecumbency.Othercommonandpathognomonicfindingsincludeperiocularedemaandvariableedemainothersubcutaneousregions.Afecalsampledemonstratedanovergrowthofgram-negative,lactose-fermentingcolonies.Onthebasisoftheclinicalpresentation,exclusionofotherpotentialconditionscompatiblewithedemaandneurologicdiseases,physicalexamfindings,microbiologyandtheresolutionofsignsaftertherapy,thepigwasdiagnosedwithedemadisease.
