Ultraviolet (UV) radiation-induced immunosuppression enhances cutaneous carcinogenesis . Subcutaneous capsaicin (8-methyl-N-vanillyl-6-nonena-mide),,the principal pungent agent in hot peppers of the genus Capsicum, has been shown to abolish UV-induced immune suppression in mice [2-4]. However, there are no published studies on the effects of capsaicin on UV-induced immunosuppression in humans.We investigated the effects of topical 0.075% capsaicin (Sigma, Australia), dissolved in ethanol and then mixed in a cream base (VanPen; Professional Compounding Centers of America, Houston, Texas) on UV-induced suppression of delayed type hypersensitivity (DTH) responses to tuberculin purified protein derivative (PPD) in healthy Mantoux-positive volunteers. The Mantoux reaction is used clinically to assess previous exposure to Mycobacterium tuberculosis or the Bacille Calmette Guerin vaccine, and has been used previously in our laboratory as a model of UV immunosuppression in humans .
