    • 摘要: Background: Advancing the science of nursing education through technology into teaching and learning is essential to prepare nurses for the changing healthcare environment. Objective: To synthesise evidence in nursing students’ competency in the clinical setting immediately after completion of training with simulation compared with lectures. Methods: Databases were sourced electronically from CINAHL and PubMed using keywords Simulation, lecture, clinical competency and new graduates. Study Eligibility Criteria, Participants and Interventions: Published articles addressing the research objectives, written in English, spaning between September 2010 and March 2019 directly answer the PSCOT question with results applicable to the population specific-nursing students, simulation was utilized. Study Appraisal and Synthesis Methods: Extracted data from sorted studies were analysed and assessed for quality using the John Hopkins Evidence-Based Appraisal Tools. Results: The two-database yielded 520 articles, 10 met the eligibility criteria. Findings documented three important themes: 1) Clinical competence among nurses;2) Medical residents’ clinical competence and 3) Was about the competence of anaesthesiology residents. Limitations: The identified studies used small sample sizes ranging from 20 to 98 participants in the single research studies. Conclusions: The review of the literature found that the active teaching strategy of simulation was more effective in promoting clinical competency in nursing students compared with the lecture method. The evidence-based teaching project proposes the use of simulation as the primary teaching method to improve the clinical competency abilities of registered general nursing (RGN) diploma students.

    • 摘要: Martian caves have revived interest in the field of subsurface exploration because they are the potential destinations for future human habitats and astrobiological research.There are many pits on Mars,but some of them look like collapsed cave roofs.These special pits are formed by the collapse of surface materials into the subsurface void spaces.The signature of life is probable in a subsurface cave on Mars as the subsurface environment can protect life from the harsh and dangerous radiation environment of the surface.In a cave,there may be an abundance of minerals,fluids,and other key resources.Therefore,locating the access point of the subsurface cave is essential and crucial for formulating plans for robotic/human explorations of the Red Planet,Mars.We have used remote sensing data from Mars Reconnaissance Orbiter(MRO;NASA),Mars Global Surveyor(MGS;NASA),and Mars Odyssey(NASA)for identifying,mapping,and classifying selected special pit candidates on the flank of Elysium Mons,Mars.A total of 32 special pit candidates has been identified and classified based upon morphology and geological context.Out of these,26 are newly discovered ones.The thermal behavior of 23 special pit candidates confirms that the special pits are radiating heat energy at nighttime,similar to potential caves.Also,cave entrances have been detected in nine candidates using data from the Hi RISE camera onboard MRO.These sites could be important destinations for future robotic/human exploration and the search for life on Mars.

    • 摘要: Lift force of an asymmetric capacitor in arbitrary shape still cannot be precisely calculated. For an irregular surface of a general asymmetric capacitor of which electric charge is nonuniformly distributed, we dedicate effort to filling the gap of lift force calculation problem. Through theoretical derivation, a general formula result is achieved. By applying theoretical formula combining with experimental method based on similarity principle of dimensional analysis, we can almost figure out all types of lift force of asymmetric capacitor. It is really an exciting method which can universally solve the hard thrust force calculation problem completely.

    • 摘要: BACKGROUND Postoperative delirium(POD)is one of the most common complications in older adult patients undergoing elective surgery.Few studies have compared,within the same institution,the type of surgery,risk factors and type of anesthesia and analgesia associated with the development of POD.AIM To investigate the following three questions:(1)What is the incidence of POD after non-ambulatory orthopedic surgery at a high-volume orthopedic specialty hospital?(2)Does surgical procedure influence incidence of POD after nonambulatory orthopedic surgery?And(3)For POD after non-ambulatory orthopedic surgery,what are modifiable risk factors?METHODS A retrospective cohort study was conducted of all non-ambulatory orthopedic surgeries at a single orthopedic specialty hospital between 2009 and 2014.Patients under 18 years were excluded from the cohort.Patient characteristics and medical history were obtained from electronic medical records.Patients with POD were identified using International Classification of Diseases,9^th Revision(ICD-9)codes that were not present on admission.For incidence analyses,the cohort was grouped into total hip arthroplasty(THA),bilateral THA,total knee arthroplasty(TKA),bilateral TKA,spine fusion,other spine procedures,femur/pelvic fracture,and other procedures using ICD-9 codes.For descriptive and regression analyses,the cohort was grouped,using ICD-9 codes,into THA,TKA,spinal fusions,and all procedures.RESULTS Of 78492 surgical inpatient surgeries,the incidence from 2009 to 2014 was 1.2%with 959 diagnosed with POD.The incidence of POD was higher in patients undergoing spinal fusions(3.3%)than for patients undergoing THA(0.8%);THA patients had the lowest incidence.Also,urgent and/or emergent procedures,defined by femoral and pelvic fractures,had the highest incidence of POD(7.2%)than all other procedures.General anesthesia was not seen as a significant risk factor for POD for any procedure type;however,IV patient-controlled analgesia was a significant risk factor for patients undergoing THA[Odds ratio(OR)=1.98,95%confidence interval(CI):1.19 to 3.28,P=0.008].Significant risk factors for POD included advanced age(for THA,OR=4.9,95%CI:3.0-7.9,P<0.001;for TKA,OR=2.16,95%CI:1.58-2.94,P<0.001),American Society of Anesthesiologists score of 3 or higher(for THA,OR=2.01,95%CI:1.33-3.05,P<0.001),multiple medical comorbidities,hyponatremia(for THA,OR=2.36,95%CI:1.54 to 3.64,P<0.001),parenteral diazepam(for THA,OR=5.05,95%CI:1.5-16.97,P=0.009;for TKA,OR=4.40,95%CI:1.52-12.75,P=0.007;for spine fusion,OR=2.17,95%CI:1.19-3.97,P=0.01),chronic opioid dependence(for THA,OR=7.11,95%CI:3.26-15.51,P<0.001;for TKA,OR=2.98,95%CI:1.38-6.41,P=0.005)and alcohol dependence(for THA,OR=5.05,95%CI:2.72-9.37,P<0.001;for TKA,OR=6.40,95%CI:4.00-10.26,P<0.001;for spine fusion,OR=6.64,95%CI:3.72-11.85,P<0.001).CONCLUSION POD is lower(1.2%)than previously reported;likely due to the use of multimodal regional anesthesia and early ambulation.Both fixed and modifiable factors are identified.

    • 摘要: This paper deals with some parabolic equations,where the reaction and the boundary flux are taken of some general forms.We study the explicit blow-up time estimates according to the different coupled relationship,including the lower and upper bounds of blow-up time for every dimension of space domains.As examples,the results could be used to so many completely coupled models.

    • 摘要: Objectives: Osteoarticular pathology has benefited greatly from MRI. This modality was only recently introduced in our country and its implantation is still rare. The objective of this study was to evaluate the practice of MRI of osteoarticular system in Douala General Hospital. Method: It was a cross-sectional descriptive study during a five-month period from May to September 2017 involving all patients referred to the Imaging Department of the Douala General Hospital for osteoarticular MRI. All the patients were scanned using an open-sided mid-field MRI APERTO LUCENT (0.4 T) using axial, sagittal and coronal slices in T1-weighted, T2-weighted proton density. T1 gadolinium or water-fat saturation (WFS) slices were obtained after injection of a contrast agent (Dotarem). Collected data were analyzed using SPSS v20 and Microsoft Excel 2010 softwares. Results: 34 patients were enrolled in this study;osteoarticular MRI accounted for 12% of the activity behind MRI of the spine (40.4%) and the brain (42.5%). The mean age of patients was 37.6 ± 13.1 years with extremes between 6 and 61 years. There was male predominance with a sex ratio of 1.83. Trauma (44.2%) and pain (42.1%) were the most common indications. MRI of the lower limb (85.3%) and more particularly of the knee (68.9%) was more frequently performed than that of the upper limb (14.7%). Orthopedic surgeons (38.2%) and rheumatologists (26.5%) were the main referral physician. 73.5% of request’s forms did not conform to the eight compliance criteria according to the recommendations of French National Agency for Accreditation and Health Evaluation. Most of the examinations (94.1%) were performed without the injection of contrast agent (Dotarem). 97% of MRI was pathological. Traumatic pathologies (39.4%) were most frequent and concerned the knee (27.3%) and the ankle/feet (12.1%), followed by the degenerative pathologies (21.2%) which concerned the knee (18.2%) and hip joints (3%), inflammatory pathologies (12.1%) reaching the shoulder (9.1%) and wrist/hand (3%) while tumoral pathologies (6%) were reaching the knee (3%) and the leg (3%). Conclusion: The MRI of the osteoarticular system is quite common, ranking third after the MRI of the spine and the brain. This study showed that MRI has a significative impact on diagnosis of osteoarticular diseases whit a concordance of 97% with clinic.

    • 摘要: In 1916, F.S. Macaulay developed specific localization techniques for dealing with “unmixed polynomial ideals” in commutative algebra, transforming them into what he called “inverse systems” of partial differential equations. In 1970, D.C. Spencer and coworkers studied the formal theory of such systems, using methods of homological algebra that were giving rise to “differential homological algebra”, replacing unmixed polynomial ideals by “pure differential modules”. The use of “differential extension modules” and “differential double duality” is essential for such a purpose. In particular, 0-pure differential modules are torsion-free and admit an “absolute parametrization” by means of arbitrary potential like functions. In 2012, we have been able to extend this result to arbitrary pure differential modules, introducing a “relative parametrization” where the potentials should satisfy compatible “differential constraints”. We recently noticed that General Relativity is just a way to parametrize the Cauchy stress equations by means of the formal adjoint of the Ricci operator in order to obtain a “minimum parametrization” by adding sufficiently many compatible differential constraints, exactly like the Lorenz condition in electromagnetism. In order to make this difficult paper rather self-contained, these unusual purely mathematical results are illustrated by many explicit examples, two of them dealing with contact transformations, and even strengthening the comments we recently provided on the mathematical foundations of General Relativity and Gauge Theory. They also bring additional doubts on the origin and existence of gravitational waves.

    • 摘要: The paper is concerned with the formulation of the static problem of general relativity. As known, this problem is reduced to ten equations for the compo-nents of the Einstein tensor and the solution of these equations is associated with two principal problems. First, since the components of the Einstein tensor identically satisfy four conservation equations, only six of these equations are mutually independent. So, the set of the Einstein equations actually contains six independent equations for ten components of the metric tensor and should be supplemented with four additional equations which are missing in the original theory. Second, for a deformable solid the Einstein tensor is associated with the energy tensor which is expressed in terms of six stresses induced by gravitation. These stresses are not known and the relativity theory does not propose any equations for them. Thus, the static problem of general relativity cannot be properly formulated because the set of governing equations is not complete. In the paper, the problem of completeness of the general relativity governing set of equations is analyzed in application to the spherically symmetric static problem and the proposed approach is further described for the general case. As an example, linearized axisymmetric problem is considered.

    • 摘要: In this article we modify our previous model for the mechanisms underlying the glitch phenomena in pulsars. Accordingly, pulsars are born with embryonic cores that are made of purely incompressible superconducting gluon-quark superfluid (henceforth SuSu-cores). As the ambient medium cools and spins down due to emission of magnetic dipole radiation, the mass and size of SuSu-cores must grow discretely with time, in accordance with the Onsager-Feynmann analysis of superfluidity. Here we argue that the spacetime embedding glitching pulsars is dynamical and of bimetric nature: inside SuSu-cores the spacetime must be flat, whereas the surrounding region, where the matter is compressible and dissipative, the spacetime is Schwarzschild. It is argued here that the topological change of spacetime is derived by the strong nuclear force, whose operating length scales are found to increase with time to reach O (1) cm at the end of the luminous lifetimes of pulsars. The here-presented model is in line with the recent radio- and gravitational wave observations of pulsars and merger of neutron stars.
