International epidemiological data suggest that urolithiasis is increasing in incidence and prevalence and progressively becoming a major public health problem. Recent studies have suggested that up to 40% of recurrences can be prevented by rational urinary stone metaphylaxis reducing the economic and social burden of this disease. Detailed knowledge of stone composition and medical history of the patient is a prerequisite for a proper metaphylaxis. Therefore, in 2010, Unimed-BH, a private non-profit health care organization operating in Belo Horizonte, Brazil, started a payment for quality project (P4Q) on prevention of chronic kidney disease. Objective: Describe the prevalence of urolithiasis at Unimed-BH and a P4Q strategy to encourage prevention and reduce the rate of recurrence of kidney stones. Results: Currently Unimed-BH serves 1 million people; 12.262 of whom with the diagnosis of urolithiasis. From 2008 to 2011, there were 59,073 emergency room admissions and 9,836 hospitalizations directly associated with urolithiasis. Trie mean length of hospital permanence at Unimed-BH was 2.45 days per patient. Conclusions: This initiative was Unimed-BH's fust step in order to increase our patient's satisfaction and reduce the burden of chronic conditions as urolithiasis and CKD.
