Ultrasound in the assessment of the bowel

Ultrasound is a well established imaging technique for many abdominal and pelvic conditions, such as gall-stones, liver abscesses or fibroids. Readily available, it is present in all imaging departments, but many have yet to realise its full potential. It is still not standard practice to teach sonographers and radiologists to look at the bowel, although this has been documented as a valuable technique since it was first described over 20 years ago (almost as soon as ultrasound began to be used in clinical practice). There are many modalities used for assessing the bowel, including barium, CT and MRI. Many patients, however, do not present with a clear label and must undergo a number of investigations in order that their condition may be evaluated. Patients with cholecystitis, urinary tract infection or fibroids require assessment of the gall-bladder, kidneys or uterus, and it is impossible not to look at the bowel while scanning these organs. One of the frequently used arguments for modern CT scanning is that it enables the radiographer/radiologist to look at other areas if the suspected diagnosis, such as renal colic, is not proven. This is no less true for ultrasound. For all these reasons, healthcare professionals involved in abdominal imaging should have just as much awareness of, and interest in, the appearance of bowel pathology on ultrasound as they have of other organ pathology. This article illustrates several common conditions that present in general medical practice to show the ease with which bowel pathology can be identified through the use of ultrasound.
