In 2017, the French Regional Pharmaco- vigilance Centre in Nord-Pas-de-Calais reported a case of photosensitivity after cutaneous application of ketoprofen, a nonsteroidal anti-inflammatory drug (NSAID) (1). A 52-year-old woman presented at the Emergency Department with a very inflammatory eczematous skin rash on her knees. She had been prescribed fusidic acid for topical application. In the absence of any improvement, she was prescribed roxithro-mycin 3 days later, and then amoxicillin + clavulanic acid after another 2 days. She was subsequently admitted to hospital in the Dermatology Department where the possible role of the keto pro fen-based gel, which she was applying to both knees for joint pain, was raised. She had started applying this gel one week before the first lesions appeared and had been exposed to the sun. After she was advised to stop applying this gel, the lesions regressed (1). Photosensitivity reactions with eczema and bullous rash, sometimes extending beyond the application period, are known adverse effects of topical ketoprofen (2). In practice Topical ketoprofen carries a greater risk of serious photosensitisation than other topical NSAIDs, which justifies its inclusion in Prescrire's review of drugs to avoid since 2013 (and also in 2018, see April issue).
